Facilitating Community Collaboration to Leverage Resources
Healthy Carolinians Partnerships are collaborations that identify health and safety issues in a community and call on existing local resources to formulate solutions that improve the health and well-being of local residents. They expand health promotion through initiatives such as developing walking trails and building fitness centers, establishing smoking prevention/cessation programs targeted to teenagers, and offering health screenings through parish nursing and worksite promotions.
The Partnerships align well with the strategic direction of the Trust, and many were established with support from previous grants awarded by the Trust. HCPs are an effective building block to facilitating, developing, and linking local efforts, particularly those that target the health and wellness needs of the financially needy.
Because the Trust recognizes the many barriers faced by local HCPs, it offered Round II funding as part of its commitment to support the continuation or expansion of existing partnerships. In particular, Round II funding provides support for the basic infrastructure of certified partnerships.
