The Kate B. Reynolds Charitable Trust is seeking proposals for an initial evaluation of Healthy Places NC (HPNC), a 10-year “place-based” initiative designed to improve health outcomes in economically disadvantaged rural communities across North Carolina. HPNC began operating in three Tier 1 counties (Beaufort, Halifax, McDowell) in May 2012.
