- View the Attachment Instructions to see a list of the attachments you will need to submit your application.
Complete the Application Forms Online:
- Visit the appropriate application webpage. Applications will only be accepted through the online application process.
- IMPORTANT: Once you create an account, you will receive an email containing your login information, including a link to your online account. SAVE THE EMAILED LINK - You must use this link (not the ones below) to return to your saved application.
HEALTH CARE DIVISION - September 15th deadline APPLICATION IS OPEN
- Fall Cycle Grants Please read our RFA announcement. This request for proposals is limited to operating fund requests from organizations based in Tier One counties, or those with an existing programmatic presence in these counties. Applications will be considered in all of the Trust's priority areas (Diabetes, Mental Health and Substance Abuse, Access to Primary Medical Care, and Community Change).
POOR AND NEEDY DIVISION - July 15th deadline DEADLINE HAS PASSED
- Second Wind Initiative Please read our RFA announcement. This request for proposals focuses on smaller Forsyth County organizations, to provide valuable program support at a critical time. Maximum grant award = $50,000. All grants are for one year.
