Operating funds to support a program that addresses the root causes of homelessness by removing barriers to employment and permanent housing called Project Cornerstone.
United Way of Forsyth County, Inc.
Programmatic support to fund the poverty mapping component of an initiative called the Family Independence Initiative (FII). The FII will develop an action plan by mapping all current initiatives addressing poverty in the community, both by location and programmatic focus, while also engaging those in poverty to provide expertise in the challenges and most effective methods of removing obstacles to economic mobility. Trust support will be primarily limited to the poverty mapping aspect of the request.
AIDS Care Service, Inc.
Operating funds for support for indigent HIV+ populations at Holly Haven Family Care Home.
Experiment in Self-Reliance, Inc.
Capital funds to purchase computing and database management systems for improved operating efficiency.
The Center for Rural Health Innovation
Capital funds to improve access to health care for low-income youth in all 14 public schools in McDowell County. The Health-e-Schools program utilizes telemedicine equipment to connect nurse practitioners to students, current equipment is antiquated and unreliable. Funds will cover 12 mobile telemedicine units in McDowell County and associated software fees.
CenterPoint Human Services
Continuation of operating funds to provide substance abuse screening and treatment services to low-income individuals in Rockingham and Stokes counties.
Rockingham County Healthcare Alliance
Capital funds to purchase equipment for a new rural health center site for low income individuals in Rockingham County.
Bethesda Center for the Homeless, Inc.
Capital funds to upgrade the computer systems/technology for mandated reporting requirements by the Carolina Homeless Information Network (CHIN) and Homeless Management Information System (HMIS).
Down East Partnership for Children
Programmatic funds to explore the technical aspects of establishing food procurement system to support farm to child care programs in Nash and Edgecombe counties.
Family Services, Inc.
Programmatic support to complete funding previously requested and granted in the Trust’s spring 2017 grant cycle. This funding was for the continued support of Family Service's Forsyth County School Readiness Project (FCSRP) which was funded as a part of the Trust's early childhood development initiative, Great Expectations. The FCSRP serves families that qualify for Head Start which is for children from financially-disadvantaged families and mostly low-income families of color. The project helps increase self-regulation and executive function in three- and four-year-old children by providing classroom management training and coaching to teachers.
