The National Health Law Program
Programmatic funds to support meaningful feedback loops between Medicaid recipients, managed care companies, and state agencies in North Carolina.
Guilford Child Development
Programmatic funds to expand Nurse-Family Partnership (NFP) services to specifically address perinatal mood and anxiety disorders. NFP is an evidence-based nurse home visitation program for low-income, first-time mothers. NFP nurses at Guilford Child Development (GCD) have all received Mental Health First Aid training to intervene in acute cases, but GCD plans to take a more sustainable approach to treating these issues by engaging a licensed professional counselor to attend initial home visits with the nurses to make the transition to mental health services smoother in cases where a need is identified. Funds will support a portion of the director’s time, supplies, supervision of counseling interns, and training.
Care Ring
Programmatic funds to educate organizations and individuals in Mecklenburg County about Medicaid reform and the transition to value-based care.
Toe River Health District
Operating funds to support a new primary care clinic targeting low income individuals in Avery and surrounding counties.
Family Services, Inc.
Programmatic support to continue the school readiness project for young children from financially disadvantaged families that Family Services started two years ago to begin the Great Expectations early childhood development initiative.
The North Carolina Public Health Foundation
Operating funds to support enhanced physician training around diabetes care and enhanced referral system development to include better diabetic patient tracking.
Bladen County Health Department
Programmatic funds to support asset mapping and strategic planning for a broad-based opioid task force in Bladen County.
Columbus County
Capital funds to add fitness equipment to an established walking trail near a farmer’s market and social service agencies in a rural, underserved section of the state. Funds will cover the costs of installing an accessible work station, equipment and building a shelter and bathrooms for use in connection with programming for students and residents who need free exercise options.
Wake Health Services, Inc.
Operating funds to continue providing substance abuse and other mental health services at Horizons Healthcare for the Homeless.
El Buen Pastor Latino Community Services
Operating support to continue funding El Buen Pastor's parent-child education programs called Listos! and Mas Listos!.
