McDowell County Health Coalition, Inc.
Programmatic funds to expand and deepen work in the four focus areas developed by the community: 1) Clinical-Community Connections, 2) Wellness Where You Live, Learn, Work, and Pray, 3) Local Food Systems, and 4) Built Environment. Funds will strengthen the connections of large institutions with community members, patients, and clients. The project will work with the Trust-supported community engagement effort currently under way to develop locally driven strategies and solutions to healthcare challenges faced by residents. Funds will support partial salary support for the executive director and cover the costs of workgroup convening's and logistics.

Operating funds to coordinate a group of medical providers in Edgecombe County who want to share data and better manage the care of high need populations.
