Programmatic funds for the School of Nursing to add a housing support specialist to the Durham Transitions Consult Clinic (DTCC) which will provide individualized care management to homeless patients. The DTCC will provide personalized assistance to homeless patients to help them identify and access available housing options. The team at Duke has determined that homelessness drives many adverse health outcomes and leads to greater emergency department utilization, longer inpatient hospital stays, and missed appointments. This proposal seeks to improve these health measures by getting patients established in stable housing. Funds will support a housing support specialist to the clinic team.
