Crosby Scholars Community Partnership

Programmatic funds to help Forsyth County students in poverty enroll in college by providing ACT/SAT Test Prep, apply for FAFSA (Free Application for Federal Student Aid), financially plan for college, and award Last Dollar Grant Scholarships to help reduce loan indebtedness. A financial counselor helps to determine which students would most benefit from a scholarship. Funds can make a difference for students enrolling in a college they might otherwise have considered financially inaccessible. Funds will cover the cost of the financial aid coordinator, the cost of the ACT/SAT test prep instruction, and Last Dollar Scholarships.

Northwest Community Care Network

Programmatic funds to implement the IMPACT (Improving Mood-Promoting Access to Collaborative Treatment)model for low-income residents of Davie, Forsyth, and Surry counties. Through an integrated care approach Northwest Community Care Network plans to screen older adults for depression, anxiety and cognitive decline/dementia and provide them with education, case management and brief therapeutic treatment. Depression among older adults is a serious health problem, often leading to unnecessary suffering, impaired functioning, increased mortality and excessive use of health care resources.
