Programmatic funds for the continued training and leveraging of low-income Rocky Mount residents to engage housing issues as a social determinant of health.
Hyde County Health Department
Programmatic funds for a Latino outreach worker to assist with interpretation during clinical care, case management, and community education in Hyde County.
Town of Aurora
Programmatic funds for a recreation coordinator based in an underserved area of Beaufort County. Sports and other activities, such as Girl Scouts, are currently organized by a volunteer committee. This group of engaged residents has spurred interest in recreation and they are asking that Aurora host a position dedicated to amplify their efforts. The recreation coordinator also will help oversee the town’s community garden and serve as the primary contact for countywide health improvement initiatives. Funds will primarily support the salary for the coordinator position.

Operating funds to support a prediabetes and obesity prevention program targeting low-income individuals in McDowell County.

Programmatic funds to support coordinated care for the uninsured in Greene, Pitt, Martin and Beaufort counties.
The B.E.L.L. Foundation, Inc.
Operating funds to expand the middle school summer school program for financially needy students in Equity Plus middle schools in Forsyth County.

Operating funds for the further establishment of a statewide PACE Association to support the successful implementation of PACE sites throughout the state.
