HNC 2030 Scorecard: Forsyth County 2021-2024

This scorecard represents Forsyth County Public Health’s Community Health Improvement Plan, highlighting progress across five key health priorities, including infant mortality and the infant mortality disparity ratio. Forsyth County’s Community Health Assessment identified goals to reduce the infant mortality and infant mortality disparity ratios as a priority. This scorecard also includes progress of the work done by Forsyth County Infant Mortality Reduction Coalition (FCIMRC).

This tool can be used to follow Forsyth County’s progress in its goal of reducing the infant mortality rate and the infant mortality disparity as well as updates from the Forsyth County Infant Mortality Reduction Coalition.
