PLACES: Local Data for Better Health
PLACES provides model-based, community-level analysis of 36 chronic-disease realated measures for all counties, states, and census tracts nationwide. As an extension of the 500 Cities Project, PLACES provides health data for small, rural communities across the country. This tool can be used by local health departments and jurisdictions regardless of the size of their populations or rurality. It is also a useful tool for developing public health interventions based on health measures distribution.
Local health departments and jurisdictions can use the PLACES tool to understand the burden and geographic distribution of health measures in their areas and plan public health interventions based on the data available.
