McDowell LFAC
Programmatic funds to support low-income, food insecure community members during COVID-19 crisis in addition to supporting systems change approaches that increase equity through the creation of new food markets and vendors in underserved areas of McDowell County.
North Carolina Association of Free Clinics, Inc.
Programmatic funds to increase sustainability for member clinics and to better position members to participate in North Carolina’s changing health care environment. Funds will support personnel to assist member clinics to fulfill the mandate that they connect to the state’s Health Information Exchange system by June 2019. Funding will be utilized to investigate, and potentially develop, arrangements that allow charitable clinics to receive reimbursement for uninsured patients and will provide partial salary support for personnel and technical assistance.
United Way of Forsyth County, Inc.
Programmatic support to fund a project called the School and Community Data-Sharing Project (SCDSP) focused primarily on Title I schools and community organizations that primarily serve financially-disadvantaged children and families as a project/program of The Forsyth Promise. The purpose of the SCDSP is to facilitate the creation and maintenance of individual-level linked data sets using a data software platform called COMET Informatics. Data will be shared with and used by approved data contributing organizations for tracking student outcome measures. Funds will support salaries and software costs related to the SCDSP.
County of Hyde
Programmatic funds to implement a comprehensive plan to combat substance misuse and opioid addiction in one of the most rural counties in the state. The deaths of several young people in Hyde County from opioid overdoses sparked the community to develop a strategic plan for reducing addiction rates that included the formation of a stakeholder task force and hosting public meetings. Residents settled on a three-part response that includes implementation of an evidence-based prevention program in the schools, increasing treatment options for people suffering from addiction, and employing harm reduction strategies. Trust funds, along with county appropriations, would cover costs for a social worker and a patient navigator along with some supplies and telepsychiatry services.
Crosby Scholars Community Partnership
Programmatic funds to support students in Forsyth County prepare for graduation and post-secondary education through mentoring, coaching and affinity programs.
The Walking Classroom Institute
Operating funds to implement the Walking Classroom in Beaufort, Burke, Edgecombe, Halifax, McDowell, and Nash counties.
Care Ring
Programmatic funds to build stronger connections between health systems and community-based organizations in underserved areas of Charlotte.
The Conservation Fund
Programmatic funds to build the capacity of small nonprofits that are connected to vulnerable communities in Burke, Robeson, Columbus, and Bladen counties to increase the ability of marginalized populations to participate in health improvement efforts.
YMCA of Western North Carolina
Operating funds to implement a diabetes treatment program utilizing components of the YMCA Diabetes Prevention Program and ADA Recognized Diabetes Self-Management Program.
McDowell Technical Community College
Programmatic funds to serve as a backbone organization for resident groups in underserved areas of McDowell to engage in broad-based health improvement efforts.
