Programmatic funds to increase sustainability for member clinics and to better position members to participate in North Carolina’s changing health care environment. Funds will support personnel to assist member clinics to fulfill the mandate that they connect to the state’s Health Information Exchange system by June 2019. Funding will be utilized to investigate, and potentially develop, arrangements that allow charitable clinics to receive reimbursement for uninsured patients and will provide partial salary support for personnel and technical assistance.

Programmatic support to fund a project called the School and Community Data-Sharing Project (SCDSP) focused primarily on Title I schools and community organizations that primarily serve financially-disadvantaged children and families as a project/program of The Forsyth Promise. The purpose of the SCDSP is to facilitate the creation and maintenance of individual-level linked data sets using a data software platform called COMET Informatics. Data will be shared with and used by approved data contributing organizations for tracking student outcome measures. Funds will support salaries and software costs related to the SCDSP.

County of Hyde

Programmatic funds to implement a comprehensive plan to combat substance misuse and opioid addiction in one of the most rural counties in the state. The deaths of several young people in Hyde County from opioid overdoses sparked the community to develop a strategic plan for reducing addiction rates that included the formation of a stakeholder task force and hosting public meetings. Residents settled on a three-part response that includes implementation of an evidence-based prevention program in the schools, increasing treatment options for people suffering from addiction, and employing harm reduction strategies. Trust funds, along with county appropriations, would cover costs for a social worker and a patient navigator along with some supplies and telepsychiatry services.
