Western Piedmont Council of Governments
Programmatic funds to increase access to care for low-income individuals by developing a fixed bus route in Burke County. The proposed route will link Morganton, Valdese, and Rutherford College, as well as Carolinas HealthCare Blue Ridge-Morganton and Carolinas HealthCare Blue Ridge-Valdese. The bus route will help reduce the number of missed medical appointments while also providing reliable transportation to many major employers, a variety of educational facilities, and several full-service grocery stores. Funds will help finance the final portion of bus line operations.
Piedmont Health Services, Inc.
Operating funds to support clinical services at a new PACE program site located in Pittsboro serving low income seniors in Chatham, Lee and Orange counties.
NC Institute of Medicine
Operating funds to develop a North Carolina rural health action plan.
Isothermal Planning and Development Commission
Programmatic funds to develop a community food and health hub in McDowell County that seeks to address food insecurity issues for low-income residents, foster economic development in the local farming community, and improve health outcomes for individuals by increasing consumers’ knowledge and confidence to use more fresh, healthy foods in their daily meals.

Programmatic funds to expand a youth engagement program that seeks to increase access to healthy foods in underserved areas.
Crossnore Communities For Children
Programmatic funds to engage in a second phase of planning to create a traumatic resilient community in Forsyth County to mitigate the impact of adverse childhood experiences and toxic stress.
Pender Alliance for Teen Health
Operating funds to provide mental health services to adolescents in Pender County through two school-based health centers.

Operating funds to enhance diabetes self-management and treatment for depression among low-income individuals in Montgomery, Richmond, and Hoke counties.
McDowell County Schools
Capital funds to support the physical enhancement of eight low-income elementary schools to promote physical activity and the afterhours/weekend utilization by the community.

Operating funds to support community-wide active and healthy living programs targeting low-income individuals in Hyde County.
