This county level report explores 21 social determinants of health that the state will focus on for the Healthy NC 2030 initiative. This report includes data on disconnected youth and data that can track ACEs. Health NC 2030, led by North Carolina Institute of Medicine and NCDHHS, collaborated with experts and leaders to develop a common set of public health indicators and targets for the state. This report serves as a north star for counties and localities.
