Urban Institute | Upward Mobility Initiative
The Upward Mobility Data Dashboard displays mobility metrics for indicators identified as the strongest collective predictors of mobility from poverty.
Opportunity Index: How Opportunity Measures Up In Your Community
This interactive data tool provides a snapshot of 4 dimensions of opportunity that can be used to identify and improve access to opportunity -economy; education; health; and community. Data is included for Forsyth County; North Carolina; and the U.S. Youth disconnection is included as one of the community indicators.
Forsyth County | Healthy Communities
This county level report explores 21 social determinants of health that the state will focus on for the Healthy NC 2030 initiative. This report includes data on disconnected youth and data that can track ACEs.
Nourishing Forsyth Latine
This report describes a community researcher-led survey that was done to understand the awareness level of ACEs, toxic stress, and trauma in the Latine community in Forsyth County.
National Health and Nutrition Examination Survey
The National Health and Nutrition Examination Survey (NHANES) measures the health and nutritional status of adults and children in the United States and includes both interviews and physical examinations. In addition to demographic, socioeconomic, dietary, and health-related questions, the NHANES also contains medical, dental, and physiological measurements, as well as laboratory tests administered by highly trained medical professionals.
North Carolina Data Portal
The North Carolina Data Portal provides users with access to data, maps, and tools required to conduct community health assessments and other public health activities. North Carolina’s NC Data Portal is poised to serve as the primary source of secondary health data for most local health departments.

Indicators of Healthy Aging: A Guide to Explore Healthy Aging Data through Community Health Improvement
