We focus on outcomes, because we believe this is the most effective way to achieve long-term, sustainable change. We always start by listening to the communities we serve and seek community-led solutions, grounded in racial equity and systems change.

Obesity Reduction in Beaufort County
Like many parts of the state, rural Healthy Places NC communities are working to address high obesity rates and food insecurity. To assist stakeholders in Beaufort in building a more equitable food system, the Trust will support efforts aimed at implementing local action plans.
Obesity Reduction in Halifax and Beaufort Counties
Like many parts of the state, rural Healthy Places NC communities are working to address high obesity rates and lack of access to active living resources. To assist stakeholders in Halifax and Beaufort counties, the Trust will support efforts that could include increasing recreational opportunities in underserved areas, conducting an equitable recreation master planning process, or promoting countywide recreation coordination.
Obesity Reduction in Robeson, Bladen, and Columbus Counties
Like many parts of the state, rural Healthy Places North Carolina communities are working to address high obesity rates and food insecurity. To help stakeholders in Robeson, Bladen, Columbus, and surrounding counties build a more equitable health system, the Trust will support efforts aimed at collecting and analyzing data and creating locally relevant plans to coordinate effective interventions.
Organizational Capacity
This Healthy Places opportunity aims to provide small general operating support grants to groups with strong networks among racial and ethnic minorities or in low-income neighborhoods.
Planning and Capacity Building to Close Racial Disparities in Birth Outcomes
Every expectant family in Forsyth County deserves a healthy birth outcome for their newborn– regardless of their race or economic status. This funding opportunity is intended to help address disparities and ensure healthy birth outcomes for all families.

Promoting Equitable Health Outcomes by Addressing Social Drivers of Poor Health
We know that achieving greater equity in health outcomes will require genuine partnership between community organizations, especially those embedded in underserved areas or led by financially disadvantaged residents, and health systems and hospitals. The Trust is interested in hearing from organizations and collaboratives, particularly in rural areas, working to build these community partnerships to address a specific social driver of poor health.

Public Education on Value-Based Care
This funding opportunity is for organizations that can help educate and inform concerned residents, community-based organizations, safety net providers, and other key stakeholders about Medicaid reform and the health system’s transition to value-based care.
