Organizational Capacity
This Healthy Places opportunity aims to provide small general operating support grants to groups with strong networks among racial and ethnic minorities or in low-income neighborhoods.

Promoting Equitable Health Outcomes by Addressing Social Drivers of Poor Health
We know that achieving greater equity in health outcomes will require genuine partnership between community organizations, especially those embedded in underserved areas or led by financially disadvantaged residents, and health systems and hospitals. The Trust is interested in hearing from organizations and collaboratives, particularly in rural areas, working to build these community partnerships to address a specific social driver of poor health.

Public Education on Value-Based Care
This funding opportunity is for organizations that can help educate and inform concerned residents, community-based organizations, safety net providers, and other key stakeholders about Medicaid reform and the health system’s transition to value-based care.
Reconnecting Youth to Economic and Education Opportunities
This funding opportunity advances economic mobility and equity for Forsyth County youth (ages 16 – 24) who have been disconnected from school and work, by increasing the number of youth who move into living-wage jobs. We know that these youth—particularly youth of color—are impacted by systemic barriers and institutional racism—and need support to secure living wage jobs and find the path to economic and educational success. This funding opportunity is designed to encourage communications, advocacy, and community engagement efforts that improve outcomes for youth and young adults not connected to work or school.
